Azusa Dialysis

Azusa Dialysis Center provides compassionate in-center dialysis care for patients with Chronic Kidney Disease and End-Stage Renal Disease. Our facility accepts ambulatory patients as well as tracheostomy and ventilator-dependent patients. We work closely with hospitals, skilled nursing facilities, case managers, physicians, and families to support safe transitions of care, timely scheduling, and quality patient support in a comfortable outpatient setting.
